To participate in the SMGF project, you must fill out an SMGF Consent Form,
which gives us permission to use your DNA sample and genealogy.
The form also gives you the right to have your sample and genealogy removed from our project at any time. Please read our
Participant Privacy Policy or contact us if you have questions.
The instructions for filling out the Consent Form vary, depending on your age range:
The 7 to 12 page shows instructions for this age range.
The 13 to 17 page shows instructions for this age range.
The 18 and Older page shows instructions for this age range.
Please note that children under the age of 7 may not participate in the SMGF project at this time.
